There is a high demand for services at Manes and Miracles. Please complete the Client Interest Survey to be added to our waitlist and a Manes and Miracles representative will reach out when an opening becomes available that meets your needs. Please keep in mind that physical therapy with horses has the longest wait time at up to 3 years. Other programs may have a few months to a year wait. Thank you for your patience!

Intake Forms

Liability forms are required annually. Healthcare Provider Clearance is required every two years.

**Clients need only complete the Intake Form and Liability Waiver and the Health Care Provider Statement.**
